6 months: Most people without periodontal disease need a professional cleaning every 6 months. If there is any periodontal disease, then more often than that. Get seen by a dentist for their expert advice.

Depends: Normally at least twice a year. I realize that insurance companies are trying to limit dental visits, but some people are better off with more than 2 per year due to their hygiene or their being prone to more buildup and plaque.
