It might: It may give you acid/indigestion that may feel exactly like a heart attack. But if you are at risk otherwise, better safe than sorry and get yourself checked.

Could be: Large hiatal hernias frequently mimick heart attacks, but so can reflux symptoms and esophageal spasms or other types of motility disorders. First of all, make sure it's not a heart attack, and see a doctor asap. If its the upper GI tract, that will require fairly extensive anatomical and physiological testing to find out.
