Briefly.: A pap smear only shows individual cervical cells. In order to know if someone has mild, mod, or severe dysplasia, a biopsy needs to be done to see how far the abnormal cells go below the surface. Cervical biopsies are usually tiny samples (3mm) that are quickly taken from the cervix with a special instrument. Most women state that they feel a "pinch" but the pain lasts for only a few seconds.

For a second: Biopsies of the cervix are like a very strong cramp that lasts for a few seconds. You can take some Ibuprofen an hour before your appointment to help ease the pain.
