Carefully: You want to remove it in the same direction it went in. If you are having any degree of difficulty see a doctor to remove it. Depending on where it stabbed you anatomical structures should be examined to make sure not compromised.

Foreign body: If the tooth pick is lodged in your foot, you should disinfect it with alcohol then try to grab the end with tweezers. If the toothpick doesn't come out, please see your primary physician or podiatrist. You may also have to be placed on an antibiotic.
