Might need: Antibiotics . You did not say how long ago you stepped on it. If it just happened you could try soaking in domeboro three times a day for 15 minutes but if there is a chance of leaving a splinter behind or if it has already been a couple of days the you should probably seek care.

Yes indeed: All signs that an infection is developing. This would mean immediate attention. Entrance wounds need to be cleansed and drained. Antibiotics are required to eradicate punter wound infections. Other consideration are the amount of time since injury and tetanus shot status.
