Possibly: If you are not responding to the antibiotics then you would need to move to surgery. If your symptoms resolve on antibiotics. Then there is a 40% chance. You will not get recurrence of appendicitis so i would advise against appendectomy unless you had recurrence of your symptoms at which time appendectomy would be appropriate.

Generally, no. : If one is treated nonsurgically for appendicitis, this management may fail, requiring surgery; it wouldn't be offered as a second option a few days later. In fact, the technical challenges and complication rates associated with appendectomies are much greater in this circumstance, as compared to early operative intervention.

More risky: Surgey up front is a very safe and effective option for treating appendicitis. Antibiotics can work, but then the question will always remain, when is it going to come back? The longer you wait for surgery in the acute setting, the more risky it becomes. After a few days, best to stay on antibiotics, and consider surgery after at least 6-8 weeks.
