Recurrent appendicitis occurs in 5-38% of medically treated patients and perforations can occur in these patients. So both of your doctors are correct and you will need to decide if you prefer medical or surgical treatment.

Mostly it depends : On your physical exam. If the process has been going on for few days, you have mild discomfort and no signs of peritonitis then options are abx / + - ct guided drainage, if theres an abscess, and do elective interval appendectomy in 8-12 weeks.On the other hand if there are signs of peritonitis then you need surgery. Interval appendectomy depends on your surgeons preferance. I usually recommend it.
