Dentist and xray: Only an xray can determine the type of infection you have around the tooth. You could have a dental abcess where the inside of the tooth is infected from a fracture or a cavity. You could also have gingivitis - infection of the gums or periodontitis - infection of the bone around the gums. Sometimes a sinus infection can cause discomfort in a tooth.

Abscess: Gum disease at early and even moderate stage usually is not painful. If you can feel it, it is probably is an abscess or a severe case. Proper diagnosis must be done at the dental office. Antibiotic should be prescribed after a thorough tooth debridement (scaling and root planning), occlusal adjustment should be performed, and follow up in 4-6 weeks to ensure periodontal health is controlled.
