Possible injection?: If the xrays show no fractures and the MRI shows no significant cartilage or ligamentous injuries, i think the next step would be to try an intra-articular injection of cortisone and continue with a low impact quadriceps strengthening program. Only after failure of all conservative measures should one consider possible knee arthroscopy as a possible diagnostic and therapeutic tool.

Therapy: Hard to tell by your description exactly what is happening, but i would rec physical therapy. If swelling, then consider draining knee and injection joint with a steroid. Ortho doc can do this.
