I have a tear in my knee very painful mri from company dr says it could be an old injury but i never had one now my dr says it severe tear and gave me?

Knee injury: An MRI is a good scan and will show any soft tissue injuries to the knee. I would followup with the advice given by your physician. Some knee injuries may require surgery, others can resolve well with physical therapy.

Age of tear: Your current MRI shows a meniscal tear. If you have had a previous study, say a year or two ago, which shows the same tear, then it is an old tear or possible a re-tear of an old tear. If you have not had a previous MRI, and a tear is present, then it is not possible to reliably date the age of a tear. A severe tear means that your Doctor thinks you will probably benefit from endoscopic surgery.
