Rheumatic fever: Streptococcal pharyngitis if not treated may result in inflammation of the heart muscles/tissues resulting in carditis.

Depends: Strep throat itself does not damage the heart. About 1/200 who do not get treated for a strep A infection, whatever the location, can develop rheumatic heart disease which does injure the heart.
