Right leg pain: Swollen ankles can be a manifestation of congestive heart failure, dependent edema, liver problems, anemia, varicose veins etc. Right leg pain is worrisome, it may suggest blood clots, phlebitis, cellulitis, vascular disease, etc. Immediate evaluation by her primary physician is recommended, she may need a doppler studies of her legs/vascular surgery evaluation if necessary.

The differential : Diagnosis would include a possible clot in the calf or radiculopathy in the back. Either way she should be evaluated by a doctor asap.
