Lack of even a confirmatory diagnostic heart cath might lead to a path of medication choices based on presumption rather than a confirmed diagnosis. This exposes him to potential side effects as well. Regardless, current data suggests that these procedures can be done safely in the elderly.

Cad: At 80 he likely has abnormal coronaries. If he is not symptomatic on coronary artery therapy like aspirin, beta blocker, statin and maybe other, and is satisfied with his functional capacity, i wouldn't bug him. If he gets symptomatic on therapy the angio is important to see what additional therapy is possible to improve his quality of life.
