A surgeon : Can feasibly remove the urate crystals. There is a new medication given by i.V therapy called Krystexxa (pegloticase) which also does this but this is reserved for severe cases of gout that don't respond to the regular medications. And, yes, to the later question it is reversible. You will need to discuss with your doc as you need to get your uric acid levels under control.

Yes: Collections of gout crystals are called tophi. They can be removed surgically if your circulation is adequate. Thiazide is a diuretic. By removing water in your system you in effect increase the concentration of uric acid in the blood. This caused the gouty attack.

Yes and yes: There are surgical procedures to remove gout crystals but rarely performed unless the large gout crystals(tophi) are interfering with joint mobility or causing pain. Thiazide induced gout is a well documented side effect of taking that medication and usually will not reoccur unless one takes a similar medication again.
