Culture, change abx: If it is a draining ear infection, the specialist will typically culture the infection to determine the antibiotics most appropriate for the infection. If it is not draining or culture is not possible, your doctor may change antibiotics to help treat it. Either way i would see a specialist to determine the next course of action.
