Nasal steroids: There are some studies that show noticeable decrease in size and symptoms of airway obstruction in children with large adenoids who have been on nasal steroids and singulair (montelukast). This over a 3 month period. If you are hesitant it's not a bad thing to try. The effect of surgery on the otherhand is immediate (most parents wish they had done it sooner).

Depends on cause: If it is chronic infections, then one can treat with antibiotics for each infection. If there is repeated infections, then one can continue to treat with antibiotics or surgery. If it is due to airway blockage, many will try nasal steroid sprays, and anti-allergy medication to decrease the size of the tonsils and adenoids. If that is not effective, then typically surgery is recommended.
