I have a friend (60+) with swollen ankles. is there a scan or test to check for clots within the ankle area?

No need : Typically, bilateral ankle swelling is not a typical sign for blood clots. It would be highly unlikely for this person to have occluding blood clots in both legs at the same time. Other causes should be ruled out by his physician, so please get to the doctor asap.
