Call your doctor: What you are describing could be hematoma or infection. Without being able to examine you it's difficult to understand what you mean by "large" so it would be best to contact your surgeon.

See your surgeon: If you had surgery 1 week ago, you should have a follow up appointment with your surgeon soon. Call his/her office and describe what you have. They will determine if they need to see you soon and move up your appointment. If it gets worst or cause problem breathing, call the surgeon and go to the er.
