Need evaluation: You need a complete neurologic workup to see what may be the cause or causes of your symptoms. Testing after a complete history and physical exam should include: MRI of at least cervical & possibly brain, carotid/vascular studies, blood testing(b12/folate etc) and electrodiagnostic tests. Ekg as well to check for cardiac issues. The testing depends on your health history & physical evaluation.

Adjacent level: It is very possible that you are having one of the adjacent non-fused levels degenerating. Especially after 30 years. This can cause pinching of nerves leading to arm pain as well as neck pain. You need a physical exam, new xrays and an MRI of the neck by a surgeon to evaluate your neck and decide on a treatment strategy.
