Clinical diagnosis: There are really no specific tests to confirm fibromyalgia at this time, although research studies have demonstrated pet scan and CSF changes in pts with this problem. The diagnosis can be considered when there is pain and tenderness in all 4 quadrants of the body, associated with trigger points, A.M. Stiffness, headaches, sleep disorder, vague cognitive complaints, etc.

A physical exam: The diagnosis of fibromyalgia is based on criteria: you must have pain >3-6 months both above & below the waist, both left & right sides of the body, have tenderness over at least 11 of 18 defined tender points, as well as at least one non-pain symptom (fatigue, tmj, irritable bowel, others). There is no blood test or imaging study: just the exam of a skilled & knowledgeable physician.

No direct test: ? about confirmatory test for fibromyalgia (FM). No tests or imaging studies to diagnose FM. However, physical exam can discern FM pattern: asymmetric shoulders, signs of TOS, functional scoliosis, hypermobility, Gillette, Fortin, & Short Leg Signs, depression, extreme fatigue, multiple painful trigger points, muscle wasting & weakness, & obesity. Eval of these signs & symptoms via sensitive exam.
