Referral: Once your kidney clearance(known as gfr) is below 20 percent, then you can be referred to a transplant unit for evaulation for a new kidney by your nephrologist.

Your kidney doc: Hi. Your kidney doc, who is following you for your deteriorating kidney function, will discuss transplant with you. There are criteria you must meet to be considered for transplant (no terminal illness, no smoking, compliance with medication regimens, etc). You will be referred to a transplant team who will coordinate the process if you qualify. Good luck!
