Concussion: Concussion does not cause ear/ nose bleeds. Ear/ nose bleed after head trauma implies serious head bleed whereas concussion means just a head bruise.

Ear canal laceration: Bleeding from the ears can be either external (from the outer canal) or internal (behind the ear drum). With a concussion, the blow to the head can commonly cause a laceration to the external canal which may bleed afterwards. If the ear drum is ruptured and the bleeding is from behind it, that would require an ear exam, preferably by an ent.
