See a specialist: If you have not been to a spine or pain specialist, now is the time. You are already requiring opioids to adequately manage you pain. Therapy, injections, medications and surgery are all options if the MRI abnormalities fit your symptoms. Find a specialist.

See a back surgeon: Find a good back doctor you can trust, either an orthopedic surgeon or neurosurgeon, who specializes in backs. Sounds like you've had the work up. The last thing i'd recommend is a narcotic except for immediate post surgical pain. You'll get hooked on those things and breaking yourself of that habit will be worse than the back pain! exercise to strengthen your back muscles.
