Insurance: Generally yes. However never let a insurance company dictate your health needs. Only you and your dentist can decide what is the best course for you. You should have it done regardless, if you can financially afford it.

No: Most dental procedures do not need pre-approval from the insurance. However, even with a pre-authorization it's never a guaranty of payment to your dentist. Most dentists will have a policy stating that the ultimate financial responsibility is yours. So before any treatment is started i always suggest you ask how much is the work to be completed in case the insurance does not pay.
