Should be seen by Dr: If there is swelling and skin peeling, your neighbor should go see a doctor to see if there are other issues with infection.

Possibly: Due to the blistering, i would recommend seeing a physician for an evaluation to see if the bite is becoming infected or early necrosis (dying tissue which can be due to the brown recluse family of spiders or the hobo spider).
