Poor prognosis: Monoclonal gammopathy unspecified significance(mgus) does not present with the need for a blood transfusion every 2 weeks. He has progressed to myeloma or has myelodysplastic syndrome or some other serious blood disorder. Goals of care should be discussed, as he will not be able to be typed and crossed formore blood eventually. Referral to palliative/hospice care is probably appropriate.

Diagnosis?: Does not sound like mgus. Are you referring to his hemoglobin as 5? Also what infusion? There is usually no treatment needed for mgus.
