Almost never: I have seen this one time in 26 years of practice, and that was in a "street person" who had neglected the bcca on his face so long that it had grown into the brain. Otherwise the chances of this tumor causing death are tiny.

Almost never: Basal cell typically doesnt metastasize. It only spreads locally.

Rare: Basal cell can be locally destructive and cause deformities or loss of function (ie: spread to eyes, ears, brain and affect function) but rarely results in death. Unlike melanoma or squamous cell which are frequent causes of death.
