Vague question: There are many causes for a high wbc. The type of WBC that is in excess is important to know. If it is mostly neutrophils (pmns), chronic inflammation may be the cause. Since there was a bone marrow examination 5 years ago, i suspect this is the case. Smokers with bronchitis or emphysema may have somewhat elevated wbcs.

Myeloproliferative : Sounds like a myeloproliferative neoplasm. Given the fibrosis, I am sure they checked a jak-2 mutation. Difficult to make any recommendation on treatment without an examination and review of the labs.
