Depends: I guess you are describing a hiatal hernia. Small hiatal hernias are commonly seen on x-ray and have no meaning or concern unless there is heartburn or reflux problems. Can be watched or treated with antacid medication. Upper endoscopy with a gastroenterologist if you are having symptoms of frequent heartburn or swallowing problems.

Depends: If small and asymptomatic you don't need anything done. If have significant gerd or pain from a large hernia or pulmonary complications from the hernia you may need this repaired surgically.

Probably nothing: The hiatal hernia found on UGI is very common and usually doesn't mean much unless it is associated with significant physical or histologic changes in the lower esophagus. If you are overweight, it would be a good reason to take off some pounds. The hernia is agrevated by increased abdomenal pressure.
