LASIK surgeon: There are number of good lasik surgeons close to you. I would look up lasik surgeons close to you online and choose one which looks to be respected/ experienced. I would not recommend traveling to have surgery far from your home since you should have surgery with someone that is readily available to you locally if you should have any postoperative issues.
