Surgery vs PT: Sounds like you have foraminal narrowing at c5-6 and c6-7 causing symptoms of pain, headaches, affecting back, arm, hand. If you have been treated unsuccessfully with pt, injections, then your options are limited to more of the same ineffective treatment, or, if appropriate, cervical fusion. Find a physician who can explain your problem to your satisfaction, then decide who should treat you.
