Yes: There are a few ways to diagnose osteo . From my experience, bone biopsies are done less these days because imaging modalities like MRI are so sensitive.

Yes: Sampling the bone is arguably the best method, you can take half and send to pathology and the other half to microbiology for culture to find out the offending organism.

Biopsies: Yes. But the patient should be removed from antibiotics for few weeks, then cultures and specimen should be obtained. Otherwise will get false results.

Yes: Biopsy confirms diagnosis by identifying inflammatory cells in region. Sometimes bacteria isolated. Osteomyelitis can be acute, subacute, and chronic.Biopsy usually done when diagnosis is in question or need to know with certainty type of organism for proper therapy.
