What do they do at the er if you've been in a car accident and your neck hurts?

X-ray : X-rays would be a good start. See if you broke anything. Depending on other symptomoogy the er doc may want further diagnostic testing, but routinely an x-ray at minimum, in addition to a thorough clinical exam.
