It varies: There are reports that indicate that gastric bypass is superior to sleeve as far as long term weight loss is concerned. But there are obviously surgeons that get great loss with the sleeve. The bottom line is that both procedures are simply tools that must be added to lifestyle changes to get the greatest benefit. The more the tool helps to change a person's eating and exercise habits the bette.

Both similar: The sleeve and bypass work in almost all patients. Our experience is to expect about an 80% excess body weight loss with the bypass and 70% with the sleeve. So if you weigh 250 and you are supposed to weigh 150, then you have an excess weight of 100 lbs. Most patients make their decision based on side effects or risks. Both are great.

Very similar: Weight loss with both operations is in the range of 60-80% excessive weight loss. The risk profile for both operations is the same too. The only thing that leans me toward recommending a gastric bypass more than a sleeve would be severe reflux or long standing type ii diabetes. These two medical conditions seem to be better managed after gastric bypass.
