Don't you normally get drugs to set a broken bone (lets say ankle) prior to a doctor putting on a cast?

Depends: Not all broken bones need to be "set". Some are non-displaced. In that case they don't.

Some times: If only a cast is being applied and the bones to not have to be re-aligned medication would not be used.
