Chronic congestion: Yes- the need for surgery with this history is common - long-term blockage usually involves anatomical problems like deviated septum and/or enlarged turbinates and/or polyps. Most people with chronic trouble have unsuccessfully tried numerous medications. You need to see ent.

Maybe: You may have a deviated septum, or nasal polyps that may require surgery. Get a ct scan xray of your sinuses and see an ENT doctor for full evaluation.
