Go to a spine clinic: The best approach to back pain is at multidisciplinary clinics where they have physiatrists (rehab), surgeons, and physical therapists--most people don't need surgery!

Neither: You first need to see your pcp to establish a proper diagnosis. If necessary you will be referred to the proper specialist.
