Obtain a copy of : Your medical records, including a copy of the most recent prescription and bring them to your new eye doctor you find in your new home town.

Explain: Try explaining to the receptionist that you are moving and would like to be seen. If it doesn't work your options are see a different doctor or see a new one when you move.
