Hard to know: Poking sensation in heart does not sound worrisome for heart disease especially in 35 yo female. Benign nature would be supported by lack of exertional symptoms and negative family hx of structural disease. Would be worthwhile to have ekg and a good listen to ensure nothing is amiss, but at end of day, this sounds to be more musculoskeletal or perhaps esophageal.
