Intestinal transplan: It is a surgical procedure, where the liver, small bowel and usually the pancreas that are malfunctioning, are replaced with the same organs from a donor.

Both organs: This type of transplant is referred to as a multivisceral transplant. The gastrointestinal tract from the stomach through the small intestine is transplanted directly into the patient. The liver is left attached to the duodenum. You could have a separte liver transplant and isolated small bowel (you would leave off the stomach, duodenum and pancreas) this is much less common.
