Let see if i can hel: When of the things could be the problem that he have ligament injury which did not healed. Other things could be have a small fracture of the cartilage in the talus did not healed. Check it out, both could be diagnose with MRI of the area.

Sometimes there can be divots in the cartilage on the surface of the talus that can lead to continued pain a long time after the injury. An MRI can show if something like this is going on.
