Not ALS: Many reasons for tongue twitching, and best to have a dentist evaluate first. Medications could be a culprit. Unless you have muscle weakness, atrophy, and diffuse muscular fasciculations in middle age, you do NOT have ALS

Yes!: tremor or twitching of the PROTRUDED tongue is not of any major importance you do not diagnose an important rare disease on such a basis ?!!
