My tongue gets twitches/tiny spasms at the tip, while at rest. its been happening for 3+ months a couple times per day. no weakness or other issues. 28 yr old male. can these ever be benign, or do they always indicate something more serious like als?

Isolated "twitching/tiny spasms at the tip" of the tongue lasting for 3 months would never be a sign of ALS, especially in a 28 yo. Check in with your dentist and have this diagnosed.
