Get checked: Children can be difficult to read and in my experience seeing the child face to face can bring out other medical issues that may not be present such as ear infection, pneumonia. I would recommend seeing your doctor to examine your child thoroughly.

Take him to the: emergency room! A respiratory rate of 42 at his age is high. He needs an exam by a doctor and possibly an Xray and treatment. A steroid nebulization may not work quickly enough. A bronchodilator nebulization would be better. Part of the increased respirations may be from the fever, but I wouldn't take a chance. Have him checked, please.
