What can cause recurrent gallstones in the bile ducts and liver every year when gallbladder was removed years ago?

2 causes: Stones that are found in the bile duct within 2 years after removing the gallbladder are considered to have originated in the gallbladder and passed into the bile duct but not found during the operation. A stone in the duct that is found > 2 yrs post op is considered a primary bile duct stone. The treatment is the same, though--usually ERCP, sometimes with stent placement.
