Failed BMT: Please provide more details. The answer to this is dependent on many factors which you did not provide--what type of transplant was done, who was the donor or was it the patient's own cells, what specifically was the cause of failure (recurrent cancer, bone marrow never grew, loss of the graft).

Depends: If patient can tolerate it another transplant from same or another donor can be done. The causes for failure and underlying disease has to be determined. If primary graft failure antibodies have to be checked.
