Part of Mono: The hallmarks of Mono include fatique, enlarged lymph nodes and pustular tonsils. Other symptoms include fever, malaise and enlarged spleen. Mono: It is very common for tonsils to have a purulent exudate or pus on them with mononucleosis. Your lymph nodes in your neck will be large and tender, you may have trouble swallowing. If you have trouble breathing or swallowing your own spit that is an emergency and you need to go to the ER right now. Sometimes your spleen can get large and painful, so left upper tummy pain is an emergency too.
