Tough one: I'm often faced w/ pts. w/ AM & PM pain, but little during day.Should still make sure there's not another problem like UTI.Barring that, most frequent cause is neurologic or musculoskeletal.Not enough space, here, but tiredness increases lumbar pain (tired muscles) & muscles tighten during sleep if nerves inflamed, ligaments torn or vertebral dysf.Osteopathic exam/manipulaton/prolotherapy
