What would cause sudden pain and swelling on the top of left foot and ankle, no known injury and no heart prob.

Two causes that come: to my mind, without any injury (fall or something heavy falling on the foot or ankle) are Gout or Pseudogout. Others include maybe an insect bite. See your PCP to have a blood test and even some fluid removed from the swollen part might provide an answer(Dx).
