Self diagnosis: is very difficult. Physicians have spent years in training and practice to help you sort this out. Consultation with your general practitioner, a psychiatrist or a psychologist would be quite helpful.

See physician: Please see your physician for either evaluation or referral to someone who can properly diagnose you. It is possible that you have one of these conditions, a combination, or something completely different. In addition, treatments vary for each of them. Best of luck.
